George Furbank has been ruled out of England’s Nations Championship opener against South Africa after a late appendicitis scare, forcing Steve Borthwick into an immediate full-back rethink at Ellis Park.
The Times reported that Furbank was taken to hospital in Johannesburg after suffering painful symptoms following England’s captain’s run. The setback lands less than 48 hours after England confirmed the Northampton Saints back in the No.15 shirt for his first Test start since November 2024.

Furbank’s withdrawal changes the shape of England’s back field as much as the name on the team sheet. Borthwick had selected him to give Fin Smith a familiar club connection, a second distributor outside the frontline and a counter-attacking option against South Africa’s kicking pressure.
With Freddie Steward already carrying an ankle issue, Marcus Smith is now the obvious emergency full-back option if England decide they need a playmaker rather than a specialist aerial presence. That would also alter the bench balance, where Smith had initially offered late-game cover and tempo.
The match is listed by World Rugby for Saturday at Emirates Airline Park, with England opening the new competition against the reigning world champions.
